This is a Word document that you can print out, fill in by hand, then mail with your check via U.S. Mail to the address listed on the form or give the form to the Membership chair at a Guild meeting.

Quilt Appraisal Form

Phyllis Twigg Hatcher will be appraising quilts at the Annapolis Quilt Show both on Saturday and

Sunday. She is a certified appraiser with the American Quilters Society and has been

appraising new (traditional and non-traditional) , vintage and antique quilts since 1999.

If you would like to arrange an appointment, please contact her at 410-571-8847 or Appointments can also be arranged at the show as time allows.

Please allow 30 minutes per quilt.


Request to Treasurer

 Used to deposit funds or obtain funds from the Treasurer.

A Word Document with four forms on one page. You can cut the page and use each form individually or turn in the entire page with only the forms you need filled in.  You can type in your entries into the form on your computer BUT you must print out as a signature is required.  These forms can be handed to the Treasurer along with any checks or cash, at Guild meetings.  Please do not send cash through the mail.
