Used to enter your quilt into the Guild's "Quilts by the Bay" Show to be held in June.  This is a one page Adobe PDF or Microsoft Word document that can be printed, then filled in, and mailed to the point of contact listed on the form.

Used to obtain insurance for quilt show entries.   Mail the completed form to the point of contact indicated on the form.

A Word Document with four forms on one page. You can cut the page and use each form individually or turn in the entire page with only the forms you need filled in.  You can type in your entries into the form on your computer BUT you must print out as a signature is required.  These forms can be handed to the Treasurer along with any checks or cash, at Guild meetings.  Please do not send cash through the mail.
