The Point of Contact for Logo Items is Marie Murphy and her contact information is printed on the order form.  The Order Forms can be mailed to her or handed to her at a Guild meeting.

Used to become a new member of the Guild or to renew your membership.

This is a Word document that you can print out, fill in by hand, then mail with your check via U.S. Mail to the address listed on the form or give the form to the Membership chair at a Guild meeting.

 

 
   
   

Quilt Show 2014 Entry Form

 

 

Used to enter your quilt into the Guild's "Quilts by the Bay" Show to be held in June. 

Mail the form to Jeanne Hawn or hand to her at a Guild Meeting.  Her mailing address is printed on the form.

Quilt Show Country Store Inventory Form

Used to identify items you will be providing to the Country Store. 

An example is provided on how to fill in the form.

Reproduce pages 2 and, as needed, to list all your items.

 

Quilt Insurance Form

 

 

Used to obtain insurance for quilt show entries.   Mail the completed form to the point of contact indicated on the form.

Request to Treasurer

 Used to deposit funds or obtain funds from the Treasurer.

A Word Document with four forms on one page. You can cut the page and use each form individually or turn in the entire page with only the forms you need filled in.  You can type in your entries into the form on your computer BUT you must print out as a signature is required.  These forms can be handed to the Treasurer along with any checks or cash, at Guild meetings.  Please do not send cash through the mail.
