  Workshop Registration Form

Used to sign up for an AQG workshop.  It also reminds our members of the cancellation policy.  (Updated September 2007)

Membership Form

Used to become a new member of the Guild or to renew your membership.

This is a Word document that you can print out, fill in by hand, then mail with your check via U.S. Mail to the address listed on the form or give the form to the Membership chair at a Guild meeting.

 

 
  Quilt Show 2008 Entry

Used to enter your quilt into the Guild's "Quilts by the Bay" Show to be held in June.  This is a two page Adobe PDF document that can be printed, then filled in, and mailed to the point of contact listed on the form.

Quilt Insurance Form

Used to obtain insurance for quilt show entries.   Mail the completed form to the point of contact indicated on the form.

 

Request to Treasurer  Used to deposit funds or obtain funds from the Treasurer.

A Word Document with four forms on one page. You can cut the page and use each form individually or turn in the entire page with only the forms you need filled in.  You can type in your entries into the form on your computer BUT you must print out as a signature is required.  These forms can be handed to the Treasurer along with any checks or cash, at Guild meetings.  Please do not send cash through the mail.

 

Spring Retreat 2008 Form

 

Used to sign up for the Guild's Retreat.  Microsoft Word format which can be filled in, printed, and given to the Retreat Coordinator in person at a Guild meeting, mailed to her address, or emailed to her as an attachment.
