Used to become a new member of the Guild or to renew your membership. This is a Word document that you can print out, fill in by hand, then mail with your check via U.S. Mail to the address listed on the form or give the form to the Membership chair at a Guild meeting.

Used to sign up for the Guild's
Spring Retreat. Microsoft Word format which can be filled in, printed, and given to the
Retreat Coordinator in person at a Guild meeting, mailed to her address, or
emailed to her as an attachment.

Used to enter your quilt into the Guild's "Quilts by the Bay" Show to be held in June. Email the form to Laura Hack. Or mail to her address which is listed on the form. Entries are due by 15 April 2011.

Quilt Show Country Store Inventory Form

Used to identify items you will be providing to the Country Store. An example is provided on how to fill in the form. Reproduce page 2, as needed, to list all your items. | ||

Used to obtain insurance for quilt show entries. Mail the completed form to the point of contact indicated on the form.

Used to deposit funds or obtain
funds from the Treasurer.
A Word Document with four forms on one page. You can cut the page and use each form individually or turn in the entire page with only the forms you need filled in. You can type in your entries into the form on your computer BUT you must print out as a signature is required. These forms can be handed to the Treasurer along with any checks or cash, at Guild meetings. Please do not send cash through the mail.
